Comparative the efficacy of McKenzie versus Myofascial release technique for cervical radiculopathy: a brief review

Abstract
Neck pain is a widespread musculoskeletal disorder that is linked to high disability and socioeconomic costs. Cervical radiculopathy (CR) is a neurological complication that occurs due to the compression or irritation of the cervical nerve roots that can be brought about by herniation of the intervertebral disc, cervical spondylosis, or the development of osteophytes. It is typified by emanating arm pain, sensory and motor impairments. The most common conservative management methods include the use of the spinal mobilization as well as manipulation, but other forms of therapeutic methods are also being considered. The McKenzie Method lays stress on mechanical diagnosis and directional preference exercises as the means of centralizing symptoms, whereas Myofascial Release (MFR) is based on the use of myofascial trigger points elimination to relieve pain and enhance mobility. Although they are used clinically, the comparative evidence of the two approaches on CR management is scanty. To conduct a comparison of the effectiveness of the McKenzie Method and Myofascial Release techniques in patients with cervical radiculopathy with reference to their effect on the levels of pain, disability, and clinical outcomes in general. A comparative interventional study was done on patients who were diagnosed with cervical radiculopathy. The participants were randomly divided into two groups, with one of them receiving McKenzie Method and the other receiving Myofascia Release therapy. The two groups were treated within a given time (e.g. 4-6 weeks). The measures of outcomes were pain intensity (measured in Visual Analog Scale), functional disability (Neck Disability Index), and range of motion. Assessment of effectiveness of intervention was done through pre- and post-intervention assessment. The McKenzie Method and Myofascial Release technique showed a significant reduction in pain and functional ability in patients with cervical radiculopathy. Nevertheless, McKenzie Method was more effective in centralizing symptoms and functional outcome in the long term whereas Myofascial Release was mostly effective in muscle tightness reduction and temporary pain alleviation. The findings indicate that the two methods are effective since their application together or condition- specific may maximize treatment outcomes. The paper provides useful evidence that physiotherapists can use to make the right choice of interventions and, thus, improve clinical judgment and patient treatment.
